What is Transition Planning?

IDEA 2004 defines Secondary Transition planning as a coordinated set of activities for a student receiving special education services which is designed as an outcome-oriented process, that promotes movement from school to post-school activities to begin no later than age 16, which may include:
- Post-secondary education
- Vocational education
- Integrated employment
- Continuing/Adult education
- Adult services
- Independent living
- Community participation
In Transition Planning, age 16 is the deadline, not the starting point.
Transition Planning is based upon:
- Student’s needs
- Student’s strengths
- Student’s preferences
- Student’s interests
Transition services include:
- Activities needed to assist the student in reaching post-secondary goals
- Courses of Study
Who can participate in Transition planning?
- The Student
- The Parents or Guardians
- The Special educator
- The Guidance Counselor
- The CTE teacher
- The Vocational and/or Career Evaluator
- The Therapist
- Adult Agency service providers
- Anyone who has a vested interest in the student’s post-secondary activities

Transfer of Rights/Age of Majority

- Transfer of parental rights in special education that occurs when a student with a disability reaches the Age of Majority
